Documents Required in AYUSH Counselling 2025: Updated Checklist is extremely important for every student who  is preparing to get admission in programs like BAMS,BHMS,BUMS,BSMS or BNYS . The process is not just limited to qualifying NEET, but the presence of correct documents at every stage of counselling determines your success. Many students do well in NEET but are denied admission because their documents are incomplete or not prepared on time.

Whether counselling  is conducted by AACCC or by any state government, document verification is mandatory at every stage. If any document is found incomplete, in wrong format or expired, your admission can be cancelled even after seat allotment. Hence a systematic and updated checklist is a must for every aspirant.

Role of NEET Scorecard and Admit Card

NEET Scorecard is the most primary and mandatory document to participate in any type of AYUSH counselling. It not only certifies your eligibility but is also required at every stage from counselling registration to final admission.

Important points related to NEET Scorecard:

  • Download it in PDF format from the official NTA website.  
  • Your roll number, percentile, score and qualifying status should be clearly written on the scorecard.
  • Keep 2–3 colour copies and a soft copy of this document ready.

NEET Admit Card:

  • Many counselling bodies (like AACCC or some states) also demand admit card at the time of verification.
  • Admit card should have your photo and signature.

Many times students keep only the scorecard and get confused when asked for the admit card. So keep both the documents safe and organized.

10th and 12th Marksheet and Certificate

10th and 12th marksheet and certificate are mandatory as proof of academic qualification.

10th Documents:

  • Date of Birth is accepted as proof.
  • Name, parent's name, and board name should be clear.

12th Documents:

  • We certify your subject eligibility (PCB group).
  • Physics, Chemistry, Biology are compulsory for programs like BAMS/BHMS.
  • Aggregate percentage should be as per NEET eligibility.

Make sure that there are no cuttings or mistakes on the mark sheets. If there is a grading system, keep the conversion certificate with you.

Necessity of Domicile Certificate

To participate in the counseling of state quota, it is mandatory to submit the domicile certificate of the respective state. This certificate proves that the student is a permanent resident of that state and is eligible for admission in the state quota.

Who should get it made:

  • Students who want admission in 85% quota seats of a particular state.
  • Whose schooling or residence is in that state.

Key Documents:

  • Official domicile certificate issued by the state government
  • Supporting documents: ration card, electricity bill, residence proof, आदि

The domicile issuing authority of each state can be different, such as SDM office, tehsil or citizen service center. Online application facility is also available in many states.

The Domicile certificate must clearly mention the applicant's name, address, and issue date. This certificate is usually valid for one year.

Often students are deprived of this certificate during counselling due to lack of it. Therefore, start getting it made after the 12th examination so that verification can be done in time.

Category Certificate (SC/ST/OBC/EWS)

If you belong to a reserved category, you have to submit the category certificate at the time of counselling. This certificate not only proves your identity but also validates the reservation benefits you get.

Who needs it:

  • SC/ST/OBC/EWS category students who want to avail reservation benefits.

Key Terms:

  • The certificate should be of the latest financial year.
  • Must only be issued by competent authority (SDM, Tehsildar, or district magistrate).
  • The format should be one that is accepted by the Central/State counselling authorities.

Non-creamy layer status should also be clearly written in OBC-NCL certificate. SC/ST certificates must have caste name and seal of issuing authority. Family income and asset declaration is necessary for EWS certificate.

If the certificate is old or in the wrong format, it can be rejected during counselling. So get the certificate made on time and keep updating it.

PwD Certificate (if applicable)

If a student falls under the Physically Disabled (PwD) category, he/she must have a valid PwD certificate issued by a government authorised medical board.

Who needs:

  • Those students who select PwD category in NEET application and want to take reservation on the same basis in counselling.

  • Certificate should be issued by designated disability assessment centres only.
  • Disability type, percentage and validity should be clearly mentioned in it.
  • Reservation is applicable only for disability of 40% or more.

Approved Institutes:

  • Many AIIMS, government medical colleges or regional centres are authorised for disability assessment.
  • Date of issue of certificate should be 6–12 months before counselling (state-wise rules may vary).

Many times students bring certificates made by a general physician, which becomes invalid during counselling. Therefore, get a PwD certificate made only from a recognized centre and check all the details thoroughly.

Valid Photo ID Proof

A valid identity card (Photo ID Proof) is mandatory to participate in AYUSH Counselling 2025. This document verifies your identity and makes your presence legal at every stage of the counselling process.

Accepted ID Cards:

  • Aadhaar Card
  • Voter ID
  • PAN Card
  • Passport
  • Driving License

Key Terms:

  • The ID card must be original
  • Name, photo and date of birth should be clearly mentioned in it
  • The name on the identity card should be as per the NEET application

Biometric verification is done at many counselling centres, which requires photo ID proof. In such a case, laminated or damaged ID is not considered valid.

So make sure you have a clean, readable and up-to-date photo ID that can be used throughout the counselling process without any interruption.

Gap Affidavit (if applicable)

If you have taken a gap of one or more years after the 12th examination, you have to submit a Gap Affidavit in counselling. This document makes it clear that there is no place for any illegal activity or falsification in your academic career.

When is it necessary:

  • When there is a gap of 1 year or more between your last academic certificate and counselling.

Headlines:

  • Gap Affidavit should be on non-judicial stamp paper (₹10 or ₹20).
  • The reason for the gap should be clearly written in it like – entrance preparation, medical issue, personal responsibility etc.
  • There must be a self-declared affidavit and it must be attested by a local notary or magistrate.

Many counselling authorities ask for a gap affidavit to ensure that the student was on a break for genuine reasons. Keep this affidavit ready in two photocopies along with the original.

Character Certificate, Migration और Transfer Certificate

Character Certificate and Migration Certificate are the documents that reflect your past academic record and institutional conduct.

Character Certificate:

  • It is issued by your last attended school or college.
  • This certificate shows that the student's conduct in the institute has been satisfactory.

Migration Certificate:

  • This is necessary when you are taking admission from one board or university to another institute.
  • This certificate is provided by the board to the students who have studied from CBSE, ISC or State Boards.

Transfer Certificate (TC):

  • This is obtained from the school/college where you last studied.
  • This signifies that you have duly left that institution.

All these certificates are used during verification. If your current board/institution provides these in digital format, then a print out of them should be taken along with the QR code or verification number. These certificates make a professional academic profile, so keep them ready.

Documents required for NRI/OCI candidates

If you fall into the NRI or OCI category and wish to participate in AYUSH counselling, you need some additional documents. These documents prove your citizenship, contacts, and financial status.

  • Valid Passport (of the candidate or their parent/guardian)
  • OCI card or certificate (if applicable)
  • Sponsorship letter (which clarifies who is taking the financial responsibility for admission)
  • Embassy Certificate (to confirm country and permanent residence of guardian)
  • NRI bank account statement या financial proof

Other Guidelines:

  • All documents must be in English or with a translation.
  • Counselling authorities may ask for different documents, so read the state-wise and central rules carefully.
  • Copy of passport and sponsorship letter must be self-attested.

Many times candidates assume that just qualifying NEET is sufficient, but these documents are mandatory for admission in counselling under NRI quota.

Preparation at the time of Document Verification

During the counselling process, when you reach the allotted college for reporting, document verification takes place. In this step your admission is confirmed, so this is the most important step.

Preparations required during Document Verification:

  • Carry all original documents (like NEET scorecard, admit card, marksheets, certificate etc.)
  • Keep 3-5 self-attested photocopies of all documents ready
  • Put all documents in a folder in sequence (many colleges have physical file checking)
  • Carry passport size photographs (6–10)
  • If there is an error in any document, then bring along a correction affidavit or clarification letter

Top Tips:

  • Report on time for document verification
  • Do not prepare any document at the last minute
  • Along with the photocopy, keep a soft copy as well (on USB or Google Drive)
